What to Do for Your First Visit?

We want your first visit to Northern Valley OBGYN & Laser Aesthetics to be a pleasant and easy experience. Please bring a copy of your insurance card, along with your guarantor’s date of birth, address, and phone number, if necessary.
 
Arrive at least 30 minutes before your scheduled appointment to fill out any paperwork or please fill out any forms in advance. We're located at 
2810 17th Avenue South.

Prepare Yourself for Your First Appointment

Be sure you're able to pay any copay required by your insurance provider. For any patient age 18 years or younger, a parent or guardian must accompany them to an appointment to sign off on any paperwork required. Our clinic accepts most insurance providers; please call to verify yours is one of them.

What does an independent clinic mean?
We're an independent, privately-owned OBGYN office with no affiliations to any other health care organizations. We deliver our own babies and work really hard to serve your needs. With front row parking, lab, and ultrasound services, we strive to meet all of the Women's Healthcare needs of all ages!!
 
First time at our clinic? Do you want to fill out the registration forms before you get here?
Just download the forms, print them at home, fill out the best you can and bring to your first appointment. Remember: you still need to bring your insurance card for us to scan. Thanks!
 
I wonder if I'll have to pay copay?
Copays are a specified dollar amount payable for certain covered services that're your responsibility to the provider. Every contract is different for every patient. It's a good idea to call your insurance company to see if you're required to pay at time of your visit. We accept debit / credit cards for your convenience.
 
How do I get my records to NVOG when I've been seen at another facility?
You can contact the facility where you were previously receiving your care, fill out and sign a release of info and you can have them fax to our office at 701-738-2005 and we'll take care of it from there.
 
Can I get an appointment even without a referral?
We always welcome new patients! Please call 701-738-2004 for an appointment.

Emergency Clinic Hours

  • Monday - Thursday: 8:00 AM to 12:00 PM and 1:00 PM to 5:00 PM
  • Friday: 8:00 AM to 12:00 PM and 1:00 PM to 4:00 PM
  • Saturday - Sunday: Closed
